Arkansas state law REQUIRES that all immunizations administered to patients 21 years of age and younger must be reported to the state's immunization registry system.  

All pharmacies must enroll online to report administered immunizations.

Each individual pharmacy employee who access the registry must create their own user name and password.  It is a violation of the HIPAA regulations to let another person use your log in to access the registry, and this can be punishable by fines on the pharmacy AND individual pharmacist or pharmacy employee.

1.  To successfully enroll in Arkansas WebIZ, please complete the following steps:

   • The Pharmacist In Charge (PIC) or equivalent must fill out the Pharmacy Site Enrollment Form
   • Each Pharmacist must fill out the Pharmacist User Enrollment Form
   • Each and every user that will access any part of the WebIZ system will need to fill out the Pharmacy Technician User Enrollment Form
