Important Information About the Arkansas Department of Health Immunization Protocol
*** YOU MUST FILL OUT THE FORM BELOW EVERY YEAR TO RECEIVE THE NEW PROTOCOL EACH YEAR. ***
*** THE PROTOCOL EXPIRES EVERY YEAR ON MAY 5TH.***

Pharmacists applying for the 2021-2022 Arkansas Department of Health Immunization Protocol must follow certain conditions of use in order to maintain and use the current protocol.


Protocol may only be used for pharmacists who:

  • have been unsuccessful in finding a local physician to sign their pharmacy's protocol;
  • are in good standing with the Arkansas State Board of Pharmacy;
  • have current Authority to Administer;
  • have reviewed the Best Practices for Pharmacist-Administered Immunization Services in Arkansas;
  • have access to the state's immunization registry (WebIZ);
  • and are members of the Arkansas Pharmacists Association.


Additionally, the pharmacy must have adequate emergency supplies available when administering immunizations and must inform their medical liability insurer that Dr. Jennifer Dillaha will be signing the pharmacy's immunization protocol.

To be allowed continued use of the protocol, all participating pharmacists must:


Pharmacists applying for the 2022-2023 Arkansas Department of Health Immunization Protocol must submit the following information to the Arkansas Pharmacists Association:

  • Names and license numbers of all pharmacists who will be immunizing
  • Pharmacy name and address
  • Authority to Administer number
